If you are required to have a medical exam for United States Citizenship and Immigration Services (USCIS) for adjustment of status, please read these instructions carefully:

ALERT: Effective January 22, 2025

USCIS waives any and all requirements that applicants for adjustment of status to that of a lawful permanent resident present documentation on their Form I-693, Report of Immigration Medical Examination and Vaccination Record, that they received the COVID-19 vaccination. USCIS will not issue any Request for Evidence (RFE) or Notice of Intent to Deny (NOID) related to proving a COVID-19 vaccination.  USCIS will not deny any adjustment of status application based on the applicant’s failure to present documentation that they received the COVID-19 vaccination.

  1. Does a Form I-693, Report of Immigration Medical Examination and Vaccination Record expire?
  2. How long is my Form I-693, Report of Immigration Medical Examination and Vaccination Record valid?

April 2024, U.S. Citizenship and Immigration Services announced that any Form I-693, Report of Immigration Medical Examination and Vaccination Record, that was properly completed and signed by a civil surgeon on or after Nov. 1, 2023, does not expire and can be used indefinitely as evidence to show that the applicant is not inadmissible on health-related grounds.

In consultation with the Centers for Disease Control and Prevention (CDC), and based on advances in public health electronic notification, USCIS has determined that a Form I-693’s evidentiary value should no longer be limited to a certain period if it is properly completed and was signed by a civil surgeon on or after Nov. 1, 2023. USCIS officers have discretion to request more evidence or a new or updated Form I-693 if they have reason to believe the applicant’s medical condition has changed since the civil surgeon signed the Form I-693, or that the Form I-693 submitted does not accurately reflect the applicant’s medical condition and the applicant may be inadmissible on health-related grounds.

If an applicant’s immigration medical examinations were completed before Nov. 1, 2023, the prior policy still applies. Before Nov. 1, 2023, civil surgeons did not need to share or report certain information to the CDC electronically. USCIS has consulted with the CDC and determined that a properly completed Form I-693 signed by a civil surgeon before Nov. 1, 2023, continues to retain evidentiary value for two years from the date of the civil surgeon’s signature.  This does not apply to Forms I-693 filed by Operation Allies Welcome parolees. Their Forms I-693 retain their evidentiary value for three years from the date of the civil surgeon signature, through policy and in consultation with CDC. For more information see the USCIS Policy Manual, Volume 8, Part B, Chapter 4.

For more information, see the updated guidance in the USCIS Policy Manual.

  1. Please bring a government issued ID (preferably your passport) when you come for your appointment.
  2. Each person being examined will need positive identification.
  3. If you have any vaccination records with you, please be sure to bring them with you to your appointment. If you do not have your vaccination records, you will be required to do blood testing. If you decline blood testing, you will need to complete the vaccinations at your own risk.
  4. The examination is not covered by insurance and you will responsible for all costs including any vaccinations, lab work or chest x-rays you may need to meet all USCIS requirements.
  5. You should give at least 7 days for paperwork to be completed, i.e. DO NOT WAIT UNTIL THE LAST MINUTE TO ATTEMPT TO GET THIS PAPERWORK COMPLETED.
  6. Our office will contact you when the lab work is completed.
  7. USCIS now requires that the person completing the exam can read, write, and understand English. If not, you will need to bring an interpreter with you to the physical examination. The interpreter does not need to be certified.
